Toward Automated Detection of Documentation Inconsistencies in Electronic Health Records
The study used a two-stage LLM pipeline to find 3,460 potential documentation inconsistencies across 3,000 patient discharge summaries. It found that medication and diagnosis candidates were most common, affecting nearly 70% of admissions. The research proposes a graded ontology to categorize these errors by type and context for future large-scale analysis.
